How we do it?
For patients with Chronic Kidney Disease (CKD) Stages 3-5, AHN has programs and resources to delay (or even prevent) the progression of kidney disease, including:
CKD Screening Camps
On an annual basis, we screen hundreds of patients for kidney disease, including blood tests to determine creatinine levels. Our goal is to detect patients early, with the hope that they never need to go on dialysis.
CKD Registry
In coordination with our hospital partners, we identify patients with elevated creatine levels based on blood tests, alerting the hospital specialists and helping design interventions to keep patients off dialysis.
Integrated Care
Diabetes and hypertension are the two most co-morbidities amongst patients with kidney disease, accelerating the need for dialysis. We work closely with our hospital partners including endocrinologists and cardiologists to help manage these conditions effectively.
